The best place to go is the site www.1099fire.com, you fill out the form and the software will create the file for electronic filing for the 1099-INT.  The Tax Withholding will go to the correct box. You cant use the free fillable forms to file by paper or electronically.  If you look at those forms, they say its for "informational purposes only".

What's the best way to create a dynamic, downloadable PDF that incorporates user-entered variables?
Like Troy, I'm not aware of any WordPress plugin, or for that matter, any simple way of doing this.I've looked at a number of different options in this space and thought I might approach your questions with two options on opposite ends of the spectrum:1.  For the do-it-yourselfer there are a few tools that allow for the automated creation of PDFs.  I've experimented with alivePDF (http://alivepdf.bytearray.org/) with reasonably good success.  AlivePDF is an actionscript library that in essence allows you to "draw" PDFs.  It's powerful, but time consuming to implement.  For each letter, line, and box you need to write a bit of code effectively drawing the element  - ultimately outputting to a PDF.  It does it's job, and the final product is good - but expect to spend a lot of time tinkering to get things just right.  Particularly if the user's input is variable in length or allows for layout adjustments - you'll need to code for every eventuality which is not trivial (though doable).2.  On the other end of the spectrum is Adobe's InDesign Server.  This is the tool that was built specifically for what you're looking to do.  You create InDesign template documents with gaps/blanks that map to the data your user is providing.  An Adobe scripting language (of which there are a few flavors, but I use the one that is very much like javascript) maps between the app/user and the document.   InDesign handles a lot of the variable layout problems quite well, and ultimately is 100% automated - such that your user can create content and receive a high quality PDF with absolutely no manipulation on your end.  There's a $10,000 down-side to this since that's what an InDesign server license costs.While what you're looking to do is absolutely possible, in my experience it's not a particularly easy problem to solve.  Expect to spend either time or money (or both...) to come up with something truly robust.

A really easy (and FREE) way is using Open Office, an  open source alternative to Microsoft Office Suite.  It has an export to PDF option that I have found very useful.
